Sample Passport Information Sheet

                                                   Library Passport Program

Our library is participating in the New Mexico reciprocal-borrowing program using the Library Passport Certificates. This program enables our students, faculty, emeritus faculty, and staff to borrow materials directly from other participating colleges and universities throughout the state.

The patron will check with us before visiting another library. At that point, use the following procedures:

     1.  Ask for current, valid institutional ID.
     2.  Check our files to make sure patron is in good standing (not
           blocked for fines or overdue items).

     3.  Verify that the library the patron is interested in visiting
           participates in the program.

     4.  Fill out a LIBRARY PASSPORT CERTIFICATE, have it
          signed by authorized individual,
make a copy for your files,
          and give the original to the patron.

     5.  Make sure the patron understands his/her responsibilities
          as a borrower in this program.

     6.  Patron may request PASSPORTS to more than one library.

The patron then takes the Certificate to the participating library. The Lending Library will create records as needed, and the patron will be allowed to check out books immediately. For those Lending Libraries issuing special borrowing cards, please include Home Library name on the card.

When someone arrives at our Circulation Desk carrying a LIBRARY PASSPORT CERTIFICATE, use the following procedure: (to be compiled by each library independently)
